Labs are required for your first visit. They cannot be no more than 3 months old. If you do not have any recent labs, we can give you an order for labs or you can get them drawn at our location for a $175 fee. We will require a new set of labs every 3 months.
We accept the following insurance for your visits. Cigna, Aetna, Anthem, Humana, BSBC and United Healthcare. After your visit with the provider your medications will be sent to the pharmacy for insurance coverage. If your insurance does not cover the medication, you have the option to purchase the weight loss meds from our clinic. If you choose to opt out of insurance there is a $99 provider fee each visit. This does not include the cost of medication.
If you would like your medications shipped to you, there will be a $35 shipping fee each time your medications are shipped. Your meds will be shipped the next business day after your visit. You do have the option to pick up your meds at a time and date set by the medical assistant or provider.
An in-person physical exam and EKG are required before prescribing phentermine. EKG’s can be done at our clinic for $25. These can be covered by insurance as well.
Pricing for medications includes 4 weeks of injections, diet plan, and your choice of B12 or Lipo-C injection.
We do have telehealth services however we only accept patients in the state of Illinois and Indiana currently.
