Requirements To Participate In Telemedicine Program:
- You must be in the states of Illinois and Indiana at the time of visit. Our providers are only licensed in these states.
- Advise using a digital scale (ideally the same one) to obtain weight. During treatment try to use the same scale to maintain consistency of weight measurements.
- Also need to know your height for calculation of your BMI.
- Use cloth measuring tape to get measurement of your waist which is take that the level of your belly button.
- Recent blood pressure measurement from doctor’s office or home BP measurement
- Recent lab work in the past 3 months that include the following: complete blood count (CBC), comprehensive metabolic profile (CMP) lipid profile, thyroid stimulating hormone (TSH), Hgb A1C. This would need to be emailed to the provider prior to or day of the visit. Also, can obtain labs within 30 days of initial visit and the results to be emailed o
- You will need to be comfortable drawing up your medication to recommended amounts sand injections yourself
- Estimated time of telemedicine visit: 45 minutes.
- Provider visit fee: $30
- The visit will consist of patients providing medical and weight loss history. The provider recommended a treatment plan. Patients may be prescribed medication from a traditional or compound pharmacy. If prescribed from a traditional pharmacy, you will be able to pick it up from the pharmacy of your choice. If it is from compound pharmacy, it will be mailed to you in a relatively short period of time (3 to 7 days). A copy of your driver’s license or state ID will be needed for compound pharmacy prescriptions. Syringes will also be prescribed for pick up from the pharmacy of your choice.
- If you are found to be eligible for a controlled substance (i.e. phentermine) for your weight management plan, you must do an in person visit for a physical exam and vital sign measurement per Indiana state law.
- Payment for provider fee is due prior to telehealth visit. Payment for compounded medication must be made prior to the provider ordering the medication. A receipt will be emailed to you immediately after purchase.
Telehealth Workflow For Patients
- Call or schedule for appointment online
- Need to complete consents PRIOR to visit online. (Consent for weight loss treatment, HIPAA, Photo/Media, informed consent, consent for email use, telehealth consent)
- Ideally send requested labs prior to appointment or obtain within 30 days of initial appointment.
- Upload front and side profile pictures if you consent to photographs be taken.
- Need to enter driver’s license information on demographics info. This is needed to order prescription from a compound pharmacy
- Log on to email link of text link at scheduled time and day and begin the appointment. iPhones/iPads (use Safari) and other devices (Use Chrome or Firefox).
- After visit await arrival of prescribed medication and pick up insulin syringes from pharmacy of their choice if prescribed injections. Once received then follow directions on subscription
- You will be emailed appropriate clinic handouts for you visit.
