Our medically supervised weight loss program is a structured plan designed to help individuals lose excess body weight through a combination of dietary adjustments, FDA approved medications, and behavioral changes. These programs may vary in complexity and approach but generally include:
Comprehensive Medical Assessment
- Labs (CMP, CBC, Lipid Panel, Vitamin D, TSH, T4, Hgb A1c). Labs can be no more than 3 months old. If the patient is in need of labs they can be done in person for $175 lab fee or we can write a prescription for outpatient labs. Labs will need to be done and turned in by initial visit.
- Full medical exam including vitals, physical exam and written assesments
- Address any medical concerns
Diet Plan
Keto, Intermittent fasting, 1200, 1400 or 1600 diet. • Foucused on controlling calorie intake, improving nutrition, and fostering healthier eating habits.
Medically supervised Medications
- Compounded Semaglutide
- Prescription Ozempic, Wegovy or Zepbound
- Compounded Tirzepatide (Has currently been removed from compound purchase.)
- Prescription Phentermine (May need EKG prior to prescription)
Information You Should know Before Your Visit
- We accept the following insurance carriers: Blue Cross Blue Shield (BCBS), Cigna, Aetna, Humana, United Healthcare
- If you do not carry insurance, a provider fee of $99 is due every time you see a provider
- The provider fee is separate from medication purchase
- Patient may have a copay which will be paid on the day of prior to your appointment
- New patient paperwork will be sent via email. If you do not have access to email, please arrive 15 mins prior to your appointment to complete any paperwork or update your account
- If you are a telehealth patient (Illinois & Indiana only) your paperwork and provider fee, if applicable, is due prior to visit
- Medications can be shipped directly to your door for $35 (Shipping fee)
- If you want to make an appointment for Phentermine, an initial in-person exam is required and a EKG
- EKG’s can be covered by insurance if applicable
- Pricing for medications includes 4 weeks of injections, diet plan, and your choice of B12 or Lipo-C injection

After the provider visit, the meds are e-scribed to a pharmacy of choice for insurance approval. If approved, patient will follow up upon completion of prescribed dose. If denied:
Medications will be shipped next business day. Patients have the option to come pick up there meds at the specified med pick up time and date per the medical assistant or provider
There is a $35 shipping fee for each shipped medication For patients that opt out of insurance, there is a $99 provider each visit (In-person and Tele-health visit)
Phentermine
Phentermine has the same visit rules as Tirzepatide and seamglutide, however, phentermine requires a physical exam and EKG prior to prescription. Telehealth patients will have to schedule a in-person visit to complete this prior to prescribing phentermine.
