We truly appreciate your continued support since 2018! We’re excited to announce that effective November 1st, our office will be moving to 126 E US Highway 30, Schererville, IN 46375 — just down the same plaza, next to the dentist.
We’ve also made a few updates to improve your experience:
Phone System Update
We’ve integrated an AI assistant into our phone system to help answer common questions about pricing, services, and products. She can also provide links for gift card purchases and appointment booking.
If you’d like to speak with Brittney, NP, Bridey Hayes, or Dr. Alonzo directly, you can request to leave a message with your name and number, and the provider will return your call as soon as possible. You may also request to be transferred — if your provider is unavailable, please leave a message so we can get back to you promptly.
Check-In Process
When you arrive for your appointment, please check in at our kiosk in the lobby. If any yearly paperwork or consent forms are required, we’ll notify you before your appointment. Once checked in and forms are complete, please have a seat and your provider will call you back when ready.
Deposit & Cancellation Policy
A non-refundable $50 deposit is required for all booked appointments. This deposit is applied toward your total at checkout unless you no-show, cancel within 24 hours, or arrive more than 15 minutes late. Debit or credit cards are required for all book appointments.
For vitamin injection appointments, no deposit is required; however, a credit or debit card must be kept on file. Our cancellation policy still applies — if you miss your injection appointment, you will be charged the cost of the injection (e.g., $15 for a B12 shot).
All consultations are $50. If you decide to book your recommended service same day, this fee can be applied as your deposit.

The Rejuvenation House Team

To ensure safe and personalized care, consent forms are required before every service at The Rejuvenation House. We also require yearly forms to keep your medical and treatment information up to date.
We recommend completing your yearly forms through your patient portal prior to your appointment. If you’re unable to do so in advance, please arrive 15 minutes early to complete them in person.
Our team will notify you if your yearly forms are due once your appointment is booked. Please note that service consent forms are mandatory before each visit and must be completed to receive treatment.
Our Patient Portal makes managing your care simple and secure. Current patients can log in by clicking “Already a Patient,” while new patients can register to create an account. Through the portal, you can book appointments, securely message your provider, and upload important documents—all in a HIPAA-compliant environment for your privacy and safety.
